You are standing on the touchline on a freezing Sunday morning. The whistle blows for full time. As the team walks off the pitch, you notice it again.
Your child is limping.
They are trying to hide it, but the wince on their face every time their heel hits the ground gives it away. When they finally take their boots off in the car, they complain of a sharp, stabbing pain right at the back of their heel.
By the time they wake up on Monday morning, the pain has magically vanished. They walk to school completely fine. But after training on Tuesday night?
The limp is back, worse than before.
If this sounds familiar, you are not alone. You are likely dealing with one of the most common, frustrating, and misunderstood injuries in youth sport: Severs Disease.
If your child is aged between 8 and 12 and plays high-impact sports like football, rugby, or netball, they are right in the danger zone.
But here is the most important thing you need to know right now: Severs Disease is not actually a disease. It is completely manageable, and in most cases, your child does not even need to stop playing sport completely .
Here is the real truth about Severs Disease, why it happens, and exactly what you need to do to get your child playing pain-free again.
What Actually Is Severs Disease?
Let us get the scary name out of the way first. Severs Disease is not an illness. It is an overload condition that happens during rapid growth spurts.
When your child hits a growth spurt, their bones lengthen incredibly quickly. The problem is that their muscles and tendons do not grow at the same speed. They have to stretch to keep up.
At the back of the heel, the powerful Achilles tendon attaches directly to a growth plate (an area of developing cartilage at the end of the bone). When the calf muscles get tight during a growth spurt, they pull relentlessly on that Achilles tendon.
Now, add in the repetitive impact of high forces involved in running, jumping, sprinting, and sudden changes of direction on a hard pitch. That constant pulling causes severe irritation and inflammation at the growth plate.
That is Severs Disease. It is simply a traction injury caused by tight muscles pulling on a growing bone under high physical stress.

The 5 Warning Signs You Cannot Ignore
Children are notoriously bad at explaining exactly where it hurts. They will often just say “my foot hurts” or “my leg is sore“.
But if you watch closely, Severs Disease has five very clear warning signs that you can spot from the sidelines:
- Pain During Impact: The heel hurts specifically when running, jumping, squatting, or hopping. It is an impact-driven pain.
- The Rest Relief: The pain noticeably eases when they stop playing and rest. This is why they often feel fine the morning after a game.
- The Squeeze Test: The back or sides of the heel bone feel extremely tender if you gently squeeze them.
- The Limp: They start limping during or immediately after training to keep weight off the heel.
- Tight Hamstrings and Calves: Their leg muscles feel unusually tight. If you ask them to touch their toes, they will struggle much more than usual.
If your child is ticking these boxes, they do not need an expensive MRI scan or X-ray to diagnose it.
They need an assessment from a physiotherapist and they need smarter load management.

Why Does It Happen Now?
Growth spurts are the trigger, but they are rarely the only cause. Severs Disease usually flares up when a growth spurt collides with one of these common training errors:
- The Pre-Season Spike: Doing too much, too soon. The highest risk period is the third week of a new school term or sports season when training volume suddenly doubles after a quiet summer.
- Early Specialisation: Playing the exact same sport all year round without a break. This forces the body to repeat the exact same movement patterns relentlessly, overworking the exact same tendons.
- Lack of Strength: Weak calf, leg, and trunk muscles that cannot absorb the shock of running and jumping. If the muscles are weak, all the stress passes directly to the fragile heel bone.
- Poor Recovery: Not enough sleep, poor nutrition, and zero planned rest days. The body only repairs itself when it is resting.

The Big Question: Can They Still Play?
This is the number one question parents ask us at The Athlete Academy. And the answer is usually yes — but with strict rules.
Continuing to play with Severs Disease is not harmful in most cases, provided the pain is carefully managed. We use a simple system called the 2/10 Pain Rule.
Ask your child to rate their heel pain out of 10. If the pain stays at a 2/10 or below during activity, they can continue. It is safe to play with a tiny bit of discomfort.
However, if the pain spikes above a 2/10, lasts for more than an hour after training, or causes them to limp the next morning, you must pull them back. Continuing to push through high pain will only prolong the injury and keep them on the sidelines for months. This is not one of those “play through the pain” scenarios. That is the worst way to combat growth related issues!
How To Manage The Pain Immediately
If Severs Disease flares up and the pain hits 5/10 or 6/10, you need to act quickly to settle the inflammation.
First, take a short break from high-impact activities for two to four weeks. Swap the running and jumping for low-impact alternatives like swimming or cycling. This maintains their cardiovascular fitness without hammering the heel.
Apply ice to the heel for 10 to 15 minutes immediately after any activity to reduce swelling. Gentle calf massage can also help release the tension pulling on the Achilles.
Here is a critical warning: Never use pain medication to mask the pain so they can keep playing. Pain is the body’s warning system. If you numb it, they will overwork the injury, cause more damage to the growth plate, and make the situation significantly worse.

The Return To Sport Ladder
When the pain has settled, you cannot just throw them straight back into a full 90-minute match. You must rebuild their tolerance to impact gradually.
Follow this step-by-step progression ladder to ensure a safe return:
- Step 1: Walk
- Step 2: Swim or Bike
- Step 3: Light Jogging
- Step 4: Running
- Step 5: Jumping and Hopping
- Step 6: Sprinting
- Step 7: Change of Direction Drills
- Step 8: Return to Full Training
- Step 9: Return to Competitive Matches
The Golden Rule: Only move up to the next step if they can complete the current stage with no more than 2/10 pain for two consecutive sessions.
If the pain flares up above a 2/10, do not panic. Just drop back down a step for two sessions before trying to progress again. Recovery is rarely a straight line.
How To Stop It Coming Back
The best way to beat Severs Disease long-term is to build a body robust enough to handle the extreme demands of youth sport.
Stronger muscles protect the growth plate. Bodyweight strength training is the absolute foundation of injury prevention for young athletes. By building serious strength in the calves, hamstrings, and trunk, the muscles learn to absorb the violent impact of running and jumping, rather than passing it onto the fragile heel bone.
You must also manage their weekly schedule. Ensure they are getting enough sleep (the body only repairs itself when asleep) and eating enough high-quality food to fuel both their rapid growth and their heavy training load.
